The quote above does not include Veterinary exam fees. You will still need to pay for and see a USDA accredited veterinarian. We do not comment or arrange for pet travel to or returning from your destination. Pet logistics is not part of our services. All entry and exit fees, permits, quarantine, exams, taxes, and requirements are the responsibility of the owner where required. We advise only what is required for the health certificates from the US to your destination.
This is just an estimate for paperwork prep as required by your destination and transit countries. Rush fees for paperwork prep within 14 days are subject to a $75 fee. Outlined above denotes the services agreed to and offered.
THIS LEGALLY BINDING AGREEMENT is made in between Vet Health Docs, LLC (hereinafter “Business”) and the owner estimated in this quote (hereinafter "Client”.), the parties herein.
In consideration of the mutual promises herein contained, the parties agree as follows:
1. Health Certificates & Vaccination Records
Failure to obtain the proper health certificate and other documentation as instructed by the Business may result in travel delays. Requests made by the Business to the client must be responded to in a timely manner based on when the pet is traveling. Delays in client response are not the responsibility of the business, nor will the business be responsible for delays, cancelations, or change fees.
2. Third party clause
The business's role is to provide service to the client and veterinary office in prefilling and tracking health certificates. The business is not responsible for any delays due to 3rd party issues or errors (ex: veterinary practice forgot to submit documents, veterinary practice not linked to our USDA VEHCs account so the business cannot track, the USDA is having longer processing times, or overnight carrier has had delays in document returns).
3. Payment Terms
Payment is due in full for services selected. Additional services may be required due to changes in regulations, at which time the business will bill the client. Changes to your schedule once we have finalized your timeline will result in a $25 change fee plus any additional fees by the USDA and overnight labels. Failure to pay a change fee will result in cancelation of contract and no refund.
4. Cancellations
Client agrees that should the relocation be cancelled, for whatever reason, after initiation of this agreement, Client may be charged a cancellation fee. Cancellations made after payment and at least 30 days before the date of original travel will receive a partial refund to be determined by work performed. Cancellations made within 30 days of original travel will not receive a refund.
5. Severance of Contract Provisions
In the event that any one or more of the provisions contained in this agreement shall, for whatever reason, be held to be invalid, illegal, or unenforceable in any respect, such invalidity, illegality, or unenforceability shall not affect any other provisions contained herein, and this Agreement shall be construed as if such invalid, illegal, or unenforceable provision had never been contained herein.
6. Arbitration
Client and Business agree that any dispute arising under this agreement be resolved by binding arbitration in El Paso County, Colorado pursuant to the rules of the American Arbitration Association.
In agreeing to this quote for service I certify and acknowledge that I have had the opportunity to read this agreement, that I understand the same, and that I have voluntarily entered into this agreement fully aware of its terms and conditions.
This quotation may be accepted to form a binding contract upon selection and payment.
